# UE Prevention Rounds Handout (Pocket Version)

**Use for every intubated high-risk patient.**

## UE Risk Check

1. RASS goal documented and current sedation plan aligned.
2. Restraint status reviewed for appropriateness and integrity.
3. ETT securement method intact and position verified.
4. High-risk flags present: agitation, TBI, prior UE, repeated line/tube pulling.

## Readiness Check

1. If SBT ready and passed, plan extubation quickly.
2. If not ready, document risk-mitigation plan for next 12 hours.

## If UE Occurs

1. Complete UE event form within 1 hour (RN + RT + provider).
2. Hold bedside huddle within 1 hour.
3. Log event in monthly UE tracker.

## Daily Metric to Record

- High-risk airway checklist documented: Yes/No.
