Nursing presentation and resident assessment and plan
Version 2026-10-28
Use this sequence for each patient. Nursing presents the admission context and current bedside status; the resident interprets the findings and proposes the plan. The attending confirms decisions with the team.
Notify the bedside nurse of the rounding order. Assign a presenter and a person to enter orders and record tasks. Arrange coverage for urgent pages when staffing allows.
“This is , admitted after , on hospital day and postoperative day . The active injuries are . Major procedures include . The patient remains in the ICU for .”
Use the shared injury list and chart for accuracy. The resident confirms or adds any missing injury, procedure, or ICU indication. Keep the introduction brief once the team knows the patient.
“Overnight, . My main bedside concern today is .”
Report important changes, current support, outputs, and bedside concerns. Give exact settings or infusion rates when they affect a decision. The resident remains responsible for verifying the chart and examination.
For each active problem: current status and key evidence, interpretation, action today, and target or reassessment time.
Add relevant examination, laboratory and imaging trends, and consultant recommendations. Use the companion page as a prompt.
The resident should synthesize the nurse report and independent chart review. Discuss active problems and relevant trends; use explicit decisions for routine safety items.
